Health Services for Children with Special Needs, Inc. (HSCSN) would like to inform you about our SEAL (Screen, Educate, and Link) process. SEAL is part of the HSCSN condition management program for enrollees who are coping with health problems which are caused by traumatic events that have happened. The program is aimed at identifying your/your child’s strengths and helping you get linked to the services and support that may be needed.
HSCSN Care Managers have completed several assessments and health screenings with you/your child over time. Based on these screenings/assessments, you/your child may have support needs. Your Care Manager will assist in linking you with providers of your choice who can help meet the identified needs. Your care manager will give you options of providers who can help you decide the type of support you would like.
What do I do now?
- Expect contact from your Care Manager about providers.
- Think about services that may help you or your child.
- Share this with your Care Manager.
- Be prepared for an evaluation with your doctor or other chosen provider.
- Keep in touch with your Care Manager.
- Continue attending your other appointments.
- Ask questions! The team will work with you to get you the answers you need.
