Family-Centered Care Plan

We know the value of family in your child’s health and well-being. Our care managers work with families as partners in planning and giving care.

Each HSCSN enrollee has a Care Coordination Plan. This plan is created by a team of people, including: 

  • You (the enrollee)
  • Your family members
  • Your Care Manager
  • Your PCP and specialist(s)
  • Public agencies serving you or your child

Your Care Coordination Plan will be updated two times a year, or more often if needed. The Care Coordination Plan includes, but is not limited to:  

  • Self-directed goal(s)
  • A list of diagnoses/problems
  • A medication list
  • A list of health care providers and frequency of visits
  • Home health care services
  • Self-care
  • Durable medical equipment
  • Transitions in care
