Family-Centered Care Plan
We know the value of family in your child’s health and well-being. Our care managers work with families as partners in planning and giving care.
Each HSCSN enrollee has a Care Coordination Plan. This plan is created by a team of people, including:
- You (the enrollee)
- Your family members
- Your Care Manager
- Your PCP and specialist(s)
- Public agencies serving you or your child
Your Care Coordination Plan will be updated two times a year, or more often if needed. The Care Coordination Plan includes, but is not limited to:
- Self-directed goal(s)
- A list of diagnoses/problems
- A medication list
- A list of health care providers and frequency of visits
- Home health care services
- Self-care
- Durable medical equipment
- Transitions in care
