Medication Therapy Management

HSCSN’s Medication Therapy Management (MTM) Program helps you make sure that you are taking your medications safely. HSCSN partners with Clinical Pharmacy Associates, Inc. (CPA) to provide MTM services. For 30 years, CPA has provided pharmacy services for enrollees. CPA has a staff of advanced trained pharmacists ready to help enrollees with medication education and clinical services. Together, we will help improve your pharmacy care and service.

What is MTM?

•  It’s like a special checkup for your medicines. We’ll look at all the pills you take and make sure you’re using them correctly.
• We’ll also teach you about your medicines, like what they do and how much to take.
• You’ll have your own pharmacist. They’re like medicine experts!
• Your pharmacist will chat with you about your medicine and answer any questions you have.
• Your pharmacist will work with you to coordinate your care with your prescriber. We look forward to working with you and your prescriber to help you stay healthy through our Medication Therapy Management Program.
