Is diabetes impacting your quality of life? Are you a caretaker for a person with diabetes? HSCSN's Living Well Program is a FREE seven-week workshop to help you learn how to:

  • Set and meet realistic personal goals.
  • Manage your symptoms more effectively.
  • Deal with fatigue, pain, and decrease frustration or isolation.
  • Communicate better with your providers and make informed decisions about your treatment.

Week 1: Partnership with the District of Columbia Department of Health to offer a virtual Living Well Workshop to focus on chronic condition management, such as: diabetes, heart disease, asthma, arthritis, and cancer. Presenter: Riana Buford, MSA, MCHES Public Health Analyst, Cancer and Chronic Disease Prevention Bureau Community Health Administration (CHA). The seven-week workshop will help enrollees and caregivers learn how to: (1) Set and meet realistic personal goals, (2) Manage their chronic condition symptoms more effectively, (3) Deal with fatigue, pain, and decrease frustration or isolation, (4) How to communicate with providers and make informed decisions about health care needs about treatment. THis meeting is conducted via telephone.
