The DC MCP collaborative presents: My City, My Plan, a summer health series opportunity for DC residents to stay connected with their health plan services and community programs. The purpose of the summer health series is to host a combined MCP preventive health fair by engaging Medicaid enrollees through in-person efforts to assist with meeting their preventive healthcare needs.
