The DC MCP collaborative presents; My City my plan a summer health series opportunity for DC residents to stay connected with their health plan services and community programs. The purpose of the summer health series is to host a combined MCP preventive health fair by engaging Medicaid enrollees with in-person efforts to assist with meeting their preventative health needs.
